Understanding How Medicare Claim Works

Learning About Medicare Claim And How Long It Takes To Process Before The Claim Approves

What Is Medicare Claim

A Medicare claim is a type of payout that your insurance provider or where your insurer will pay for your medical care. Claims happen when after you see a doctor or you are treated in a hospital.


When you enrolled in a Medicare Advantage or Part D, your provider will process the claims on Medicare’s behalf.


Who File Medicare Claims

Medicare Part A and B are submitted directly to Medicare by your insurer. It takes approximately 30 days to process each claim. Medicare Part A claims directly to the facility or agency that provides you the care.

You’re the one responsible for the deductibles, copayments, and non-covered services. While Medicare Part B claims are submitted either to your provider or by yourself. But the claims will be determined by the assignments:


  • If the providers agree to accept the assignment, meaning he accepts the Medicare’s approved amount as full reimbursement. Medicare will the Part B claims directly to him or her for 80% approved amount. And the remaining 20% will be shoulder by you, this will be your co-insurance.
  • If the provider does not accept the assignment, he/she is required to send in your claims to Medicare, which then pays the Part B claim directly to you. And you will shoulder the pay 


Beneficiaries can submit their own claims for certain services when the insurer won’t file a claim. However, you cannot file a claim with the Original Medicare for diabetic test strips, Part B drugs, or medical equipment paid under DMEPOS Competitive Bidding Program. Your pharmacy or medical supplier must bill Medicare for these items.


Where Do I File Claim

Original Medicare has participating and non-participating providers. Participating providers accept Medicare reimbursement plus your coinsurance as full payment. And they agreed to always bill Medicare once you need to receive care. On the other hand, non-participating providers can charge you up to 115% of Medicare’s rate and don’t need to file claims with Medicare. This means that you have to submit your own healthcare claim if you choose to see a non-participating provider.


How Long Does The Claim Need To File

Original Medicare claims must submit within 12 months of when you received care. Medicare Advantage plan has different time limits, the time limits for advantage plan is shorter than Original Medicare. The best way to find out the time limits for your Medicare Advantage is to ask your insurance provider.

How Short-Term Health Insurance Coverage Works For Individuals With Certain Medical Condition

Have you ever wondered what it is like to buy short-term health insurance for an individual with a pre-existing condition? It is unlikely to find insurance coverage especially for people with existing conditions, but as the law changes pre-existing conditions can be covered by insurance but will be limited.


Today, we’ll talk about short-term health insurance for pre-existing conditions. How did it work for them? As we mentioned earlier insurance laws change, people with existing illnesses can buy short-term health insurance but not all enrollees can be accepted. 

Since short-term health insurance is not part of the Affordable Care Act (ACA), meaning they don’t need to comply with their certain standard, and pre-existing conditions are not covered within a short or temporary plan. However, not all pre-existing conditions disqualify you for coverage, there are some certain illnesses that will qualify you for short-term even if you have existing conditions.


Understanding Pre-Existing Conditions

A pre-existing condition is a medical problem or health condition of a person that occurs prior to applying for health or life insurance plans. Chronic illnesses and medical conditions, such as any form of cancer, diabetes, lupus, epilepsy, and depression are examples of a pre-existing condition. Pregnancy before applying for health insurance is considered a pre-existing condition.


Here is some medical condition that can affect your application for short-term health insurance:


  • Arthritis
  • Asthma
  • Cancer
  • Chronic obstructive pulmonary disease
  • Congestive heart failure
  • Diabetes
  • High cholesterol
  • High blood pressure
  • Joint replacement
  • Maternity
  • Pain constantly
  • Profound fatigue


While the lists might seem to limit, noticing that acne and sleep apnea are a few conditions that many insurance companies accept for short-term health insurance. So it is important to remember that applying for temporary health insurance can be denied or if accepted you pay an extra charge or your insurance carrier might not cover your medical treatment that correlates to your existing condition.

 

In addition, the length of time that temporary policies “lookback” for pre-existing conditions may differ by state, in most states, there’s a five-year lookback period for short-term claims. Meaning the third-party provider which will pay for your medical claims, can review your medical record from the previous five years to determine if a current condition is related or connected to your existing illness.  However, in some states, they have a shorter lookback window, if you are a resident of:

 

  • Idaho – 6 months
  • Illinois – 2 years
  • Indiana – 1 year
  • Louisiana – 6 months
  • Mississippi – 1 year
  • North Carolina – 1 year
  • North Dakota – 2 years
  • Texas – 1 year
  • Wyoming – 6 months.


The main benefit of having this coverage is repricing: if you’re uninsured, you’ll have to pay the full rate of your medical treatment while if you have short-term coverage even if the treatment is not covered, your out-of-the-pocket payment is re-priced into a lower amount to give you some financial protection. You should ask first your
insurance agent before buying this type of coverage

 

The bottom line is short-term health insurance plans are not for everyone, especially for those with existing illnesses. If you are diagnosed with one of the lists above it can be risky to buy this coverage. But if the coverage you need is for a certain amount of time, it is better to have some coverage than face and pay the full amount for an unexpected medical bill.

I sell them all. United Healthcare probably came in at the top for me this year. And there's a number of reasons for that.


The good thing about the big national companies is, they do a really good job with innovation and changing the plans, making them better.


What Does A Medicare Insurance Broker Do?


We just try to like make it easy for you to tell us, you know, your doctors, the prescription drugs that you take, and then our job is really to go out and try to find the best plan at the best rate that we can to make sure that we're taking care of your health insurance needs.

I've got if you're turning 65 you've got some different things that you need to be aware of.


Okay, and we'll walk you through that and here's just some good information when you sign up for Medicare and you know and all that a little bit about part D. So that's your prescription drugs and what what what that covers.


You got a couple of options when you're looking at Medicare you've got a Medicare supplement um you've got Medicare advantage Okay we call that part c. And then you've just got original Medicare but then you're going to be looking at copays you know and that only picks up 80% of the cost.


So you're going to be on the hook. That's why people would look at a Medicare supplement to pick up that other cost and then Medicare advantage.


So yep we're local but we can work all over the state and um we've got some good

Shopping for health insurance in Idaho is huge! Looking around the market makes you confused because they’re a lot! I mean a lot of carriers you can choose when you go to their marketplace. However, the health insurance market is dominated by five big companies.


And these companies are ranked in order according to their market share and together they controlled just about 44% of the market. And these are Anthem, Centene, UnitedHealthcare, Humana, and Health Care Service Corp (HCSC).

5 Largest Health Insurance Companies

All these carriers provide basic health insurance products to both individuals and companies, along with other insurance services that include Medicaid and Medicare policies, LTC insurance, and dental and vision benefits. To rank them as the largest companies in Idaho means that they have the highest numbers of members, or simply say covered lives . Covered lives mean the number of individuals who have enrolled in that specific carrier. 


  • UnitedHealthcare is the top health insurance company with a total of 70 million covered lives. Along with having the most members, they also have a care network of over 1.3 million physicians and health care professionals with more than 6,000 hospitals. Moreover, its policies and plans can be purchased in all 50 states. This only means that if you are a UnitedHealthcare member, you can have a wide variety of options to choose from to receive care.
  • Anthem ranks the second largest insurance provider with a covered life of over 39.9 million. Anthem is a licensee of the Blue Cross Blue Shield Association, meaning that when searching for their policies on the marketplace, you will find its products alongside BCBS followed by the name of your state. But the carrier you will be buying will be Anthem.
  • Aetna is the third-largest provider by membership with over 22.1 million members. Most of its members come from brokers and employers. Or employees planning to buy their own policies. Aetna was acquired by CVS Health Corp, an American healthcare company in 2018.
  • Cigna ranks the fourth-largest medical insurance company in the United States by members of 20.4 million. You can purchase their policies in 11 states such as Arizona, California, Colorado, and more. You can find their headquarters based in Bloomfield, Connecticut. They offer corporate health insurance policies, in addition, they also provide dental behavioral health and Medicare/Medicaid supplemental benefits. 
  • Humana comes in as the fifth-largest health insurance with covered lives of 16.6 million. Long-term care insurance is the original product before they decided to expand from selling health insurance policies. However, the company made an announcement in 2017 that they will no longer be providing individual health insurance in 11 states where they previously provided plans.


These top health insurance companies in Idaho tend to offer similar services and products to their clients, the only thing that will differ is the cost and rates. That is why I always tell my clients and customers to compare quotes from different carriers, so they will understand how they differ and they will know what is the best deal for their needs and situation also.
